Please follow the steps in order
for Passport Depot to expedite your application for a travel
visa:
Preparation of Your
Passport Application
Materials
To obtain a
TOURIST VISA, please collect
the following items:
1) Most
Recent Passport
Your U.S. passport MUST be at
least 6 months valid before the expiration date and have one
blank visa page.
2) ONE
Passport Photo
3) Complete the Tourist Visa Application Form
Please sign and complete the
Tourist Visa Application Form.
NOTE: For Question 19 of
the Visa Application Form, please check one of the item that
is the most applicable to the circumstances of your trip.
Please provide a detailed and exact description of your trip
to Brazil.
4)
ONE copy of Proof of Departure
Provide plane ticket or
itinerary showing proof of leaving the country.
5) Consular Fees
The fee for obtaining a
TOURIST VISA for Brazil is
$140.
This fee includes non-refundable processing and handling
fees. To pay this fee, please include this payment along
with Passport Depot Fees.
6) Passport Depot
Fees
Please send COMPANY CHECK, CASHIER’S CHECK, MONEY ORDER
made payable to “Passport Depot”. You may also pay this fee
by CREDIT CARD by using the
Customer Information Form.

Return Shipping Fee
To receive your passport for overnight return delivery, please
add $24.95 (or $39.95 for Saturday delivery) to your
check/money order or credit card payment to Passport Depot, or you
may include your own Airbill with your FedEx Account number.

Customer Information Form
FOR BUSINESS VISA
To obtain a
BUSINESS VISA, please include
all the following items above in addition to the following:
1) Complete the Business Visa Application Form
Please sign and complete the
Business Visa Application Form.
2) Business Letter
Please include a business letter from your employer or
sponsoring company. On their letterhead stationary, briefly
introduce the applicant (please specify employment
status/position held in the company by applicant) and
clearly state the precise nature of the business to be
conducted by applicant in Brazil.
3) Consular Fees
The fee for obtaining a BUSINESS
VISA for Brazil is $200. This fee includes non-refundable
processing and handling fees. To pay this fee, please
include this payment along with Passport Depot Fees.

After gathering all the necessary materials, please send

REJECTED IF SENT BY ANY OTHER SERVICE.
(US Postal Mail, DHL, Airborne Express and/or other major
shipping carriers’ envelopes/packaging are NOT ACCEPTED.
If so, your application will NOT be shipped appropriately
and can ultimately result in delay and/or loss of your
application materials).

location or use their
online drop-off
locator (drop-off locations will have blank
Airbills and envelopes. You may use a credit card to pay for
the sending of your application).
Inform Us of Your Order
After
completing
Stages A and B, please call Passport Depot to
inform us about your order at the toll free number

please fax a copy of the Customer Information Form to the
