Passports
You are
using this process if you have a lost or stolen passport.
Passports reported lost of stolen are invalidated and can no
longer be used for travel. Once a passport is reported lost
or stolen, it cannot be revalidated. Please follow the
instructions thoroughly to help you obtain a new U.S.
Passport.
If you meet these
qualifications, please following the instructions carefully
to obtain your NEW U.S. PASSPORT.
PLEASE
NOTE: IF YOU ARE PLANNING ON LEAVING WITHIN 3 TO 4
BUSINESS DAYS, PLEASE CONTACT PASSPORT DEPOT TO VERIFY WE
HAVE THE ABILITY TO PROCESS YOUR APPLICATION IN TIME FOR
YOUR DEPARTURE.

Preparation of Your Passport Application

blue or black ink only)

Please do NOT sign the
DS-11 application form until the
Passport Acceptance Agent
instructs you to do so.

as follows:
- Write your name as it
appeared in your passport
- Write the approximate date of issue
- Include the passport number if known
- Under “DISPOSITION”, mark the appropriate box
- If your passport was expired, write “EXPIRED” next to the
“OTHER” box

NOTE: The
information provided on the DS-64 form will be entered in
the Consular Lost/Stolen Passport System. Passports reported
lost of stolen are invalidated and can no longer be used for
travel. Once a passport is reported lost or stolen, it

$190 check or money order payable to “US Department
of State” with Date of Birth plus $35 payable to the
post office or court house you visit.
($190 for 16 and over,
$160 for under 16)

Proof of

7 ) TWO Copies of Your Proof of Departure
Provide plane ticket,
itinerary or business letter showing proof of leaving the
country.

Please print out this form first, MANUALLY fill out "Applicant Information". For
"Courier Company" line, you
MUST write "Passport Depot". Also please check the first two boxes
at the middle section of the form.

Return Shipping Fee
To receive your passport for overnight return delivery, please
add $69.00 (or $89.00 for Saturday delivery) to your
check/money order or credit card payment to Passport Depot, or you
may include your own Airbill with your FedEx Account number.

Visit a Passport
Acceptance Facility and Prepare Your Package for Shipping
You must establish your identity to the Passport Acceptance
Agent. Please inform the Passport Acceptance Agent you are
using an expediting service to "hand carry" your application
to the government passport agency for processing. After
signing the application in front of the Passport Acceptance
Agent, the PAA should seal-up the following items in their
envelope:

TWO identical Passport Photos

Proof of Citizenship - The $190 check or money order to
"US Department of State" with Date of Birth -
ONE copy of the Proof of Departure

ONE copy of the Letter of Authorization (some PAF's
may not seal the letter of authorization or the
itinerary in the envelope, if that is the case, then
place them inside the FedEx envelope along side the
sealed envelope. No delays or problems will result).
DO
NOT OPEN THE SEALED ENVELOPE OR THE APPLICATION WILL BE
INVALID.
After
visiting the Passport Acceptance Facility, please send the
following items in your package to Passport Depot:
- The sealed envelope from the
Passport Acceptance Agent
- Passport Depot Service Fee
- One
set of the filled-out
DS-11
Form
- One copy signed
Letter of Authorization
- One copy of the Proof of Departure
- The
Customer
Information Form

PLEASE SHIP BY
FEDERAL EXPRESS OVERNIGHT ONLY. YOUR APPLICATION WILL BE
REJECTED IF SENT BY ANY OTHER SERVICE.
(US Postal Mail, DHL, Airborne Express and/or other major
shipping carriers’ envelopes/packaging are NOT ACCEPTED.
If so, your application will NOT be shipped appropriately
and can ultimately result in delay and/or loss of your
application materials).

Inform Us of Your Order
After
completing Stages A and B, please call Passport Depot to inform us
about your order at the toll free number 1-800-689-1388 before 7:00
PM PST (Monday – Friday). Also, please fax a copy of the Customer
Information Form to the toll free fax number 1-800-687-1388 before
7pm PST (Monday-Friday) or email us at
